About the Role

Join Mass General Brigham as a Nurse Practitioner, Extended Care in United States. Site: Mass General Brigham Medical Group Northern Massachusetts, Inc. Key Responsibilities

Facilitating communication with the patient, the patient's family or significant other, and the referring physician

Working in collaboration with the patients' attending physicians at the SNF/LTC facility, as well as providing continued collaboration with the patients' primary care provider(s) This is a full

hour position. Our NPs/PAs are required to see 12+ patients per day when assigned to work in the Skilled Nursing Facility and/or 6

8 patients per day when performing Home Care visits. The NPs/PAs assigned to work in the Skilled Nursing Facility must be available to respond to their pagers within 15
