About the Role

Join Fraser Health as a Clinical Pharmacist, Primary Care Network - Fraser Health Community South Fraser in United States. Salary The salary range for this position is CAD $54.49 - $68.

Key Responsibilities

  • Identifies and resolves actual and potential drug therapy problems in collaboration with the patient and other members of the primary health care team.
  • Communicates pertinent findings, recommendations and plans to the patient and appropriate healthcare practitioners.
  • Consults with patients and members of the primary care health team to develop patient-specific therapeutic plans based on chart review, patient information and assessment of clinical responses.

Requirements
