Key Responsibilities

Evaluate patients using subjective data, objective measures, clinical assessment, prognosis, and function‑based goals, establishing effective and individualized treatment plans.

Implement and modify patient‑specific treatment programs to support functional progress and long‑term outcomes.

Develop and deliver patient and family education, adjusting strategies as needed to maximize engagement and understanding.

Document all clinical findings, plans, and progress in the electronic medical record in a timely and accurate manner.

Requirements

Candidates must be graduates of an accredited Physical Therapy program and possess a current or pending New Hampshire Physical Therapy license, along with CPR Certification.
