About the Role

Join PACE Southeast Michigan as a Physical Therapist – PACE Program in United States.

Key Responsibilities

  • AND FUNCTIONS: Work in conjunction with the Interdisciplinary Team to coordinate rehabilitation services for participants at PACE Southeast Michigan.
  • The Physical Therapist will perform assessments, provide treatment sessions, implement care plans, and order/issue Durable Medical Equipment.
  • Document all care provided to participants in accordance with department standards and submit to medical records in a timely fashion.
  • Maintain accurate clinical records and utilization data relevant to the rehabilitation department.

Requirements

  • one (1) year of experience with a frail or elderly population.
  • Physical ability to instruct and demonstrate exercise, provide lift assistance, and assist/manage mobility devices.
