About the Role

Join PACE Southeast Michigan as a Physical Therapist – PACE Program in United States.

Key Responsibilities

AND FUNCTIONS: Work in conjunction with the Interdisciplinary Team to coordinate rehabilitation services for participants at PACE Southeast Michigan.

The Physical Therapist will perform assessments, provide treatment sessions, implement care plans, and order/issue Durable Medical Equipment.

Document all care provided to participants in accordance with department standards and submit to medical records in a timely fashion.

Maintain accurate clinical records and utilization data relevant to the rehabilitation department.

Requirements

one (1) year of experience with a frail or elderly population.

Physical ability to instruct and demonstrate exercise, provide lift assistance, and assist/manage mobility devices.
