Key Responsibilities

• will be responsible for conducting one-on-one patient care and patient/family teaching in the home setting.

• will work directly with patients in their residence implementing therapeutic interventions to optimize their functional abilities per Plan of Care to promote safety and independence in the home setting.

• provide compassionate home-based care.

Requirements

• management, written and verbal communication skills Reliable transportation, current driver’s license and automobile insurance

• Salary: $65 - $120/visit

Work Schedule

• Per Diem position.
