About the Role

Aspire Living & Learning seeks a Community Companion Home Coordinator in Hamden, Connecticut. This role provides leadership and support to Home Care Providers, ensuring health and well-being of individuals in Shared Living arrangements.

Key Responsibilities

• Provide leadership and oversight to Home Care Providers.
• Travel extensively throughout Connecticut to visit homes and meet providers.
• Build relationships with providers and individuals supported.
• Collaborate with management, state agencies, families, and medical professionals.

Requirements

• Bachelor's degree or equivalent experience.
• Prior experience supporting individuals with disabilities preferred.
• Valid driver's license and reliable transportation.
• Strong communication and empathy.
• Basic computer skills; Therap experience a plus.
