About the Role
Aspire Living & Learning seeks a Community Companion Home Coordinator in Hamden, Connecticut. This role provides leadership and support to Home Care Providers, ensuring health and well-being of individuals in Shared Living arrangements.
Key Responsibilities
- Provide leadership and oversight to Home Care Providers.
- Travel extensively throughout Connecticut to visit homes and meet providers.
- Build relationships with providers and individuals supported.
- Collaborate with management, state agencies, families, and medical professionals.
Requirements
- Bachelor's degree or equivalent experience.
- Prior experience supporting individuals with disabilities preferred.
- Valid driver's license and reliable transportation.
- Strong communication and empathy.
- Basic computer skills; Therap experience a plus.
