Major Duties and

Key Responsibilities

Home Based Primary Care (60 to 100% of time), depending on patient panel size

Conduct comprehensive medication management for Veterans enrolled in the HBPC program, ensuring the safe, effective, and evidence-based use of medications

  • Collaborate with an interdisciplinary team to develop individualized care plans that optimize therapeutic outcomes and enhance the quality of life for each patient.
  • Provide education and counseling to patients and caregivers on medication therapy, disease state management, and lifestyle modifications.

Ambulatory Care (0 to 40% of time), depending on HBPC patient panel size

Deliver advanced clinical pharmacy services in Ambulatory Care clinics, focusing on chronic disease management, anticoagulation therapy, and other specialized therapeutic areas

  • Provide direct patient care through face-to-face or telehealth appointments, ensuring the delivery of high-quality, patient-centered care.
  • Review nonformulary medication consults to ensure appropriate, cost-effective medication therapy.
