About the Role

Serve as a Remote Patient Monitoring Home Telehealth RN Care Coordinator for Veterans in Los Angeles. Manage complex patients via telehealth technologies.

Key Responsibilities

Coordinate care delivery through remote patient monitoring.
Perform comprehensive clinical assessments.
Collaborate with interdisciplinary teams.
Provide health coaching and education.
Lead quality improvement initiatives.

Requirements

Active RN license.
2-3 years nursing experience preferred.
Case management experience preferred.

Benefits & Compensation

Salary $75,000 - $95,000 annually.
50 days paid time off (annual, sick, holidays).

Work Schedule

Monday-Friday, 8:00am - 4:30pm.
