Key Responsibilities
- Identifies and resolves actual and potential drug therapy problems in collaboration with the patient and other members of the primary health care team.
- Communicates pertinent findings, recommendations and plans to the patient and appropriate healthcare practitioners.
- Consults with patients and members of the primary care health team to develop patient-specific therapeutic plans based on chart review, patient information and assessment of clinical responses.
Requirements
