Key Responsibilities

Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.

Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record.

Conducts reassessments of patient/family needs and counseling as required.

Requirements

Candidates must hold an MSW degree from a CSWE-accredited school and possess a valid driver's license with reliable transportation. One year of experience in home health care and hospice experience are preferred.
