About the Role

Join Navajo Health Foundation - Sage Memorial Hospital, Inc. as a Licensed Clinical Social Worker – Outpatient in United States. Job DetailsJob Location: Ganado, AZ 86505Position Type: Full TimePosition Summary: Provide individual, family, and/or group therapy in an outpatient setting. Conduct substance abuse evaluations for patients seeking residential treatment and/or outpatient substance abuse treatment.

Conduct intake biopsychosocial assessments for all patients.

Create treatment plans.

Refer client to other resources within the hospital as well as in the surrounding areas.

Requirements

Master’s Degree in Social Work.

Two (2) years of progressive work experience.

Must be independently licensed as an LCSW in the State of Arizona.

Must possess a current American Heart Association (AHA) Basic Life Support (BLS) certification.

Work Schedule

Full-Time position.

Call responsibilities required.
