JOB DESCRIPTION: DALHART SENIOR CARE SOLUTIONS IS AN OUTPATIENT BEHAVIORAL HEALTH CLINIC FOR OLDER ADULTS AND SENIORS, A SUBSIDIARY OF THE DALLAM HARTLEY COUNTIES HOSPITAL DISTRICT. THERAPISTS ARE TO CONDUCT INITIAL ASSESSMENTS DETERMINING WHETHER A PATIENT IS FIT FOR THE PROGRAM.

Key Responsibilities

• Complete Biopsychosocial Integrated Assessment I & II, Social History, Cognitive Assessments, and Depression Scale with referred and current patients.

• Prepare and provide group psychotherapy why geriatric population patients requiring mental health outpatient support or other ages as license allows.

• Document therapeutic notes regarding daily group therapy patient care following session and as needed.

• Complete individual and/or family therapy as needed by the patient.

Requirements

• Master’s Degree in Mental Health field and have obtained a Texas License as a Professional Counselor of LPC

• Previous experience in healthcare of behavioral health settings preferred.

Work Schedule

• Full-time position.

• No explicit call, weekend, or night shift information provided.
