Key Responsibilities

will do Provides leadership to achieve coordination of care delivery across the care continuum for a specific patient population
Leads and facilitates efforts to coordinate multidisciplinary delivery of care that is patient-and family-centered and crosses inpatient and outpatient sites of care.
