• Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re

Requirements

• Graduate of an accredited Physical Therapy program, DPT preferred. Qualifications and Requirements

• Current, valid Physical Therapy license issued by NYS Department of Education.
