About the Role

Join Halcyon Home LLC as a Speech Therapist – Home Health, PRN in United States. Description Halcyon Home is hiring a PRN Home Health Speech-Language Pathologist to support our growing geriatric home health population in Eastern Dallas, and surrounding areas. We offer great per-visit rate and wonderful team support.

Key Responsibilities

Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).

Prepares and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner.

Analyzes patient needs and provides appropriate care that is ordered by physician as indicated in the plan of care.

Requirements

Language Pathology. State licensure as a Speech

Language Pathologist. Experience in home health care preferred. Proficiency with Homecare Homebase software is a plus. Strong communication and interpersonal skills. Dedication to patient

centered care.

Work Schedule

Per Diem position.

No explicit call, weekend, or night shift information provided.
