Key Responsibilities

• provide integrated, trauma-informed, "whole-person" care within a Patient-Centered Medical Home (PCMH) model.

• provide comprehensive, patient-centered primary care, including HIV specialty services, to underserved and v.

• provide full scope primary care across the lifespan, manage chronic diseases such as diabetes, hypertension,.

• manage a high-volume panel with compassion and empathy.

Requirements

• 3+ years of primary care provider experience.

• Previous FQHC or outpatient medical office experience (preferred).

• HIV Specialist certification (preferred).

• Bilingual in English and Spanish and/or Russian (preferred).

Benefits & Compensation

• Salary range: $102,000 to $125,000 annually.

Work Schedule

• Full-time position.

• No weekends required.
