About the Role
Form Health is hiring a full-time Physician Assistant specializing in Obesity Medicine to provide longitudinal telehealth care. Collaborate with physicians and dietitians to deliver expert, evidence-based obesity treatment.
Key Responsibilities
- Provide obesity medicine care via telemedicine.
- Collaborate with multidisciplinary team.

Requirements
- Physician Assistant degree.
- Experience in obesity medicine or related fields.
- Licensed to practice and prescribe legally.
- Prior telemedicine experience preferred.
