About the Role
Join Valley Regional Care as a MSW Home Health – Medical Social Worker in United States. We are a locally owned and operated home health agency is looking to add Nurses to its current long-term and experienced Nursing team. Operating in the Valley since 2013, Valley Regional Care has been a leading provider with 500+ patient census.
Key Responsibilities
- Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
- Carries out social evaluations and plans intervention based on evaluation findings.
- Maintains clinical records on all patients referred to social work.
- Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
Requirements
- either ACSW/LCSW licensure OR a Master's Degree in Social Work.
- Essential Job Functions/Responsibilities Assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse.
- Carries out social evaluations and plans intervention based on evaluation findings.
