About the Role
Join BAYADA Home Health Care as a Nurse Practitioner – Per Diem Hospice in United States. BAYADA Hospice is hiring a compassionate and dedicated Nurse Practitioner, (NP) to join our team. The coverage area for this position is North New Jersey Prior home care, hospice, or palliative care experience strongly preferred, but not required.
Key Responsibilities
- Making home face to face hospice recertification visits to clients in designated geographic territories.
- Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
- Following up with, executing, and properly documenting doctors' orders.
- Performing client assessments as necessary.
Requirements
- for a Nurse Practitioner (NP): A current license as a NP in New Jersey.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care, hospice and/or palliative care experience strongly preferred, but not required.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
