About the Role

Elevate Health seeks an Advanced Practice Provider (NP or PA) passionate about functional medicine. This cash-based practice in Johnstown, Colorado, emphasizes longevity and root-cause medicine with personalized care.

Key Responsibilities

Provide advanced diagnostics, genomics, IV therapy, and personalized plans.
Conduct 60-90 minute initial visits and 30-60 minute follow-ups.
Participate in weekly provider and team meetings.
Contribute to protocol development and community leadership.

Requirements

NP or PA licensure.
Functional or integrative medicine experience preferred.
