Key Responsibilities

• Provide skilled nursing and head-to-toe assessments to the adult and geriatric population.

• This includes the oversight of medication administration and wound care treatment as prescribed in the physician's plan of care.

• Provide visiting home health services to an assigned caseload including admissions, re-certification and discharges.

• Develop the Plan of Care in coordination with all health care professionals assigned to the case.
