About the Role

Join BronxCare Health System as a Social Worker – Discharge Planning, LMSW/LCSW in United States.

Key Responsibilities

Completes Social Work Psychosocial Assessments for all patients referred or in need of social services in accordance with department’s high risk criteria policy.

Demonstrates competency in identifying behavioral, cognitive, and/or mental health disorders and in recognizing the underlying factors for such disorders.

Provides supportive counseling, crisis intervention, and grief counseling to patients and their families/significant others as appropriate.

Demonstrates competency in effectively utilizing appropriate computer applications and programs within the scope of professional role and responsibilities.

MSW in Social Work

Microsoft Office

Knowledge Base Charting (KBC, Care Management (Allscripts)

Internet Navigation, Outlook
