Key Responsibilities

Implement the Social Services programs of the facility under the direction of the Administrator Participate in.

Coordinate Social Services activities with other members of the interdisciplinary team (IDT) Assist residents,.

Provide residents with information regarding resident rights, advance directives, living wills and related m.

Coordinate resources and assist residents with limited English proficiency (LEP), including verbal and written.
