Join 21st Century Home Health Services as a Part Time/Per Diem Physical Therapist Case Manager providing skilled physical therapy services in patients' homes across San Mateo County.

Key Responsibilities

Perform home evaluations and develop care plans.

Current unrestricted California Physical Therapist License.
