About the Role

BAYADA Hospice seeks an experienced full-time Registered Nurse Case Manager in Harrisburg, PA, serving multiple counties including Adams and York.

Key Responsibilities

• Conduct home visits to hospice clients.
• Administer medications, wound care, treatments, and comfort care.
• Monitor client conditions and report changes.
• Follow and document doctors' orders.
• Perform client assessments.

Requirements

• Current RN license in Pennsylvania.
• Minimum one year recent clinical experience.
• Hospice or home care experience preferred.
• EMR proficiency preferred.
• Ability to travel to client locations.
