Registered Nurse, RN- Hospice Case Manager-Full time

Imagine working full-time as a Hospice Registered Nurse Case Manager with BAYADA Home Health Care in the Brattleboro, Vermont hospice office. You will provide compassionate hospice care to patients in Wyndham, Windsor-VT, and Cheshire-NH counties.

Your duties include making home visits, administering medications, wound care, treatments, and comfort care. You will monitor patient conditions, report changes, and document doctors' orders. Occasional on-call duties after hours and weekends are required.

This position requires current RN licenses in Vermont and New Hampshire and at least one year of recent clinical experience. Prior hospice or home care experience is preferred but not mandatory.
