Key Responsibilities

• home physical therapy evaluations and treatments

• Develop and implement individualized care plans

• Complete Medicare OASIS documentation, including: Start of Care (SOC), Resumption of Care (ROC), Recertifications, Discharges

• Evaluations

Requirements

• Active, unencumbered Florida Physical Therapist license

• OASIS documentation experience required

• CPR certification

• Valid driver’s license, reliable transportation, and auto insurance
