Key Responsibilities
- Conduct and document visits – Perform and document patient and family visits in accordance with the plan of care and evolving clinical needs.
- Clinical assessment and decision-making skills consistent with current standards of advanced nursing practice.

- Minimum of 2 years of experience as an Advanced Practice Nurse; experience in hospice, home health, or a clinical care setting is preferred.
- Strong understanding of symptom management, end-of-life care, and pharmacology.
- Current driver’s license, reliable transportation, and valid auto liability insurance.
