About the Role

Join DaVita Kidney Care as a Nurse Practitioner Lead – Kidney Care, 3-Day Week in United States.

Key Responsibilities

  • provide integrated care to all ESRD patients, who are some of the most medically complex and vulnerable pati.
  • manage care for more than 20,000 patients with late-stage chronic kidney disease (CKD) and end stage renal.
  • provide great care management but we start with our heart with our patients and each other.

Requirements

  • exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hour Washington Exempt: $80,168.40/year Washington Non
