Key Responsibilities

home occupational therapy services to patients and, when applicable, in assisted living settings

Evaluate patients’ physical, cognitive, and environmental needs

Develop and implement individualized care plans in collaboration with the healthcare team

Requirements

Active Occupational Therapist license in the State of New Hampshire

Graduate of an accredited Occupational Therapy program

Minimum of 2 years of clinical experience (home health preferred)

Current CPR certification
