Key Responsibilities
- A day in the life of a Unit Based Pharmacist at Hackensack Meridian Health includes: 1.
- Validate medication reconciliation: Reconcile medications upon admission for admitted patients.
- Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care.
- Perform patient assessment and counseling: Develop individualized treatment and monitoring plans for patients.
Requirements
- Registered Pharmacist License in New Jersey 2.
- Advanced Cardiac Life Support Certification within 6 months of entering position 3.
- PGY1 and /or 3 to 5 years of Clinical experience specific to critical care Preferred:1.
- Practice experience in an Emergency room setting.

- Salary: $146K
