Job Description

About the Role

Join UW Health as a Nurse Practitioner / Physician Assistant – Home Based Primary Care in United States. Work Schedule: 80% FTE, 8 hour days, 4 days a week, core hours of 8:00am to 5:00pm. Hours may vary based on the needs of the department.

Key Responsibilities

  • Conduct comprehensive or episodic health history and physical assessment of patients/families with acute, ch.
  • Provide appropriate direction to program and clinic staff regarding patient care issues.

Requirements

  • If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
  • If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program Licenses and Certifications: Minimum
  • If Nurse Practitioner
  • Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND valid WI Drivers license and reliable vehicle
