About the Role
Join Interim HealthCare of the Twin Cities as a Hospice Medical Social Worker – M-F, No Weekends in United States. Hospice Medical Social Worker - Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. You will have independence and flexibility in the field to visit patients one on one in their home environment.

Requirements
- Hospice experience preferred
- Able to travel within geographic area serviced by office. Valid drivers license, and proof of auto insurance are required. Some functions of what you would do as a Medical Social Worker

- Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.
