Key Responsibilities

• Conduct assessments of patients' physical, mental, and functional abilities.

• Implement and monitor treatment plans, adjusting as needed.

• Provide education to patients and their families regarding safe disposition planning to least restrictive en.

• 2 years preferred Sub

• acute/inpatient rehab experience also preferred. Knowledge, Skills and Abilities
