Requirements

  • Qualifications: Current state license as a Physician Assistant or Advance Practice Registered Nurse (APRN).
  • Certification as a Physician Assistant or Nurse Practitioner (Credentialing requirement).
  • Eligible for State Controlled Substance License II through V.
  • Eligible for Federal DEA Certificate II through V.
