MTM Pharmacist at HealthCare Support (Remote)

About the Role

This is a remote Medication Therapy Management (MTM) Pharmacist position supporting CVS Health. The role involves telephonic comprehensive medication reviews for high-risk members across HI, TX, IL, FL, or PA.

Key Responsibilities

• Perform telephonic medication reviews and clinical assessments.
• Identify and resolve medication-related problems.
• Create patient summaries and prescriber communications.
• Build relationships with healthcare providers.

Requirements

• Bachelor's degree in Pharmacy or PharmD.
• Active pharmacist license in HI, TX, IL, FL, or PA.
• Experience delivering MTM using Outcomes MTM Connect platform.
• Basic computer and Microsoft Office proficiency.
• Previous call center experience preferred.

Work Schedule

• Monday-Friday, 8-hour shifts starting between 9:00a - 11:00a CT.
• Training 8:00a-4:30p CST.
