Join Bluestone Physician Services as a Nurse Practitioner / Physician Assistant - Primary Care in United States.
Key Responsibilities
- manage patients’ chronic conditions, address social determinants of health, manage transitions to and from.
- provide behavioral health support and more.
- Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities F.
