Key Responsibilities
- Will Do Conduct comprehensive psychiatric evaluations and assessments of patients’ mental, emotional, and social health needs Diagnose and treat mental health conditions using evidence-based practices Prescribe and manage psychiatric medications in accordance with Delaware licensure and regulatory requirements Develop individualized treatment plans that integrate behavioral health care with social prescribing interventions Provide patient education, counseling, and support to promote mental health stability and recovery Collaborate with care coordinators, social prescribers, and other healthcare professionals to coordinate holistic patient care Address social determinants of health by connecting patients to community resources and supportive services Promote health literacy and patient self-management through engagement with patients and their families Qualifications
Required Master’s Degree in Nursing from an accredited program Current and valid Psychiatric Mental Health Nurse Practitioner (PMHNP) certification Current and valid APRN license in the State of Delaware Certification and credentials that allow for independent practice as a behavioral health provider in Delaware Minimum of 2 years of clinical experience, preferably in behavioral health, integrated care, or community mental health settings Prescriptive authority with DEA and Delaware CSR registration Preferred Experience working in integrated behavioral health or collaborative care models Experience with social prescribing models or community resource navigation Experience working with individuals impacted by substance use disorders or recovery services Experience working with underserved or vulnerable populations Skills & Competencies Exceptional communication and interpersonal skills with the ability to build trusting relationships with diverse populations Strong clinical judgment and ability to provide patient-centered psychiatric care Ability to address health disparities and utilize a holistic approach to treatment Strong analytical and problem-solving skills Ability to adapt in dynamic healthcare environments and manage competing priorities Collaborative team mindset and enthusiasm for community-based care Commitment to equitable, trauma-informed, and culturally competent care Health Care Plan (Medical, Dental & Vision) Retirement Plan (401k, ROTH 401k) Paid Time Off 11 Paid Holidays Yearly Training & Development Wellness Resources.
