About the Role

Requirements
- Graduate of a Master’s Degree program in Social Work accredited by the Council on Social Work Education.
- Current Social Work license in the state(s) of practice, as required.
- Minimum of one year of experience in a healthcare setting; home health experience preferred.
- Demonstrates strong interpersonal, communication, and organizational skills.
