Key Responsibilities

• provide on-site care to residents in long-term care settings.

• Provide primary care services to residents in skilled nursing and long-term care facilities Perform physical.

• Develop and adjust individualized care plans Prescribe medications and monitor treatment effectiveness Colla.

Requirements

• specific license in good standing in Massachusetts Minimum 6 months of experience in long

• term care or a skilled nursing facility Strong communication, clinical, and critical thinking skills Passion for elder care and person

• centered healthcare Must be able to lift a minimum of 20 pounds and stand / walk / work on your feet up to 6

• 8 hours per day
