Key Responsibilities

• provide primary care and other services to seniors and families in our medical centers that are comfortable,.

• Provide comprehensive primary care services to adult and senior population within the Medicare Advantage/Med.

• Conduct routine and preventative visits, including annual wellness exams, chronic disease management, and ac.

• Develop and implement personalized care plans focused on improving patient outcomes, quality and satisfactio.

Requirements

• 3 years' experience providing care to Medicare and/or Medicare Advantage patients preferred Experience working in a value
