Key Responsibilities

Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders; develop and implement an appropriate plan of care following regulatory and clinical standards

Utilize standardized assessments and evidence

based practices; document progress and adjust treatments in the clinical record per

Requirements

Develop comprehensive discharge summaries and maintain regulatory and clinical documentation; record treatment minutes in the electronic system

Participate in required meetings (care conferences, utilization review, rehabilitation meetings) and complete facility

required trainings and in

Obtain necessary durable medical equipment or supplies as needed; supervise and support CFY or student SLPs; adhere to ASHA and state practice acts and codes of conduct Requirements

Salary: $117K - $118K
