Key Responsibilities

  • Actively monitor and amend patient visit schedules to maximize productivity and address patient needs, including rescheduling and load balancing; communicate changes to supervision as needed.
  • Perform comprehensive PT assessments for assigned patients, incorporating psychosocial, educational, safety, and risk factor parameters in line with regulations and APTA guidelines.
  • Develop and document a plan of care to promote functional independence and safety, with patient
  • specific goals and logical interventions advancing toward those goals.

Requirements

  • Current physical therapist license to practice in Vermont.
  • Ability to supervise PTAs, PT students, and home care aides.
  • Experience with, or strong familiarity of, conducting OASIS time points and related documentation.
  • Proficiency with required paperwork and billing processes; ability to complete forms (e. g., HHABN, Medicare notices) accurately and timely.
