Key Responsibilities

• will reevaluate nutritional care by providing home follow-up care either at the patient's residence or by phone.

• provide appropriate counseling to individuals and families/caregivers in the home setting.

Requirements

• Requirements/Qualification Indiana Certified Dietitian or Indiana Registered Dietitian.

• Master’s degree in dietitian studies from an accredited institution.

• Ability to pass background, consumer report, and drug test.

• Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Work Schedule

• Per Diem position.

• No explicit call, weekend, or night shift information provided.
