About the Role

Join Provider Partners Health Plan as a Nurse Practitioner (Weekend/Part Time) in United States.

Key Responsibilities

  • Demonstrates both autonomy and proficiency in performance of comprehensive health assessments and management of the chronically ill.
  • Collaborates with member’s primary care provider/interdisciplinary care team to create an individualized care plan focused on primary care, prevention, early detection and timely intervention.
  • Adheres to standards of documentation and collaboration as directed by the PPCM ISNP Model of Care.
  • Follows practice guidelines according to state

Requirements

  • Minimum of 2 years clinical care experience in geriatrics, long
  • term care, or internal medicine.
  • Familiarity with ISNP models of care and value
  • based care principles is preferred.
