Pharmacist, Clinical/MTM Remote, PST

About the Role

This is a remote Pharmacist position focused on clinical medication therapy management (MTM) and Part D STARS adherence programs. You will work Monday through Friday, 9am to 5pm Pacific Standard Time, providing telephonic medication reviews and collaborating with healthcare teams to optimize medication use.

Key Responsibilities

• Conduct comprehensive medication reviews and targeted outreach.
• Communicate with members and providers to improve medication adherence.
• Perform prior authorization reviews and utilization management.
• Collaborate with interdisciplinary care teams.
• Develop and implement drug utilization strategies.

Requirements

• Doctor of Pharmacy degree or equivalent experience.
• Active and unrestricted State Pharmacy License.
• ASHP-accredited residency or 2-3 years relevant pharmacy experience.
• Knowledge of clinical pharmacology and disease management.
