Join CenterWell as a Nurse Practitioner / Physician Assistant in United States.
Key Responsibilities
- manage a variety of health-related concerns, focusing on continuity of care, health maintenance, and diseas.
- Provide comprehensive care to seniors in accordance with clinical standards Collaborate daily with your care.
- Manage referrals, hospital/SNF coordination, DME, and home health services Participate in daily huddles.
- lead the huddles Document care accurately and efficiently with support from quality-based coders Particip.
