Key Responsibilities

• Manage and coordinate care for chronic conditions

Requirements

• Board certified in Family Medicine
• 5 or more years of primary care experience required, including adults and pediatrics
• Active, unrestricted Nurse Practitioner license in a minimum of 15 active state licenses required:
• – Required to have 2 of the following states: California, Texas, Florida
• – Required to have 3of the following states: Georgia, Missouri, Mississippi, Oklahoma, South Carolina, Tennessee
• At least 1 year of remote work experience preferred
