Key Responsibilities
- Lead and collaborate within an assigned Care Team (including RNs, LPNs, Medical Assistants, Behavioral Health Clinicians, and Patient Access staff) to manage the health of a patient panel.
- Champion a culture of shared responsibility for patient outcomes.
- Participate in and/or lead daily team "huddles" to review the schedule, anticipate patient needs, identify care gaps, and coordinate workflow before the clinic day begins.
