Key Responsibilities
The Physical Therapist will provide individualized skilled care to patients in their homes, assessing, interpreting, planning, implementing, and evaluating treatment programs based on patient age and diagnosis. Key duties also involve collaborating with the healthcare team, educating patients and families, and participating in agency program development and performance improvement initiatives.
Requirements
Candidates must be a graduate of an accredited Physical Therapy program and hold licensure in North Carolina or South Carolina, along with CPR certification, and possess a minimum of one year of experience. Essential requirements include the physical ability to lift over 50lbs, transfer patients over 100lbs, and possess a valid driver's license and operational vehicle.
